We ask that you bring a current photo Id and your insurance cards with you to your appointment. Please be prepared to provide a full list of all of your prescription and over the counter medications with dosage and frequency.
Also, as a courtesy to our patients with respiratory problems and migraines, we ask that you please refrain from wearing any colognes, perfumes or scented lotions during your appointment times with us.
